Well done to Avril Mc Carthy for being the first correct answer in!

“Pourrez-vous / Pourriez-vous envoyer le cadeau de ma part?”

So many pupils forgot to add the question mark at the end!  Remember, whenever you write down a question sentence, you must add ? at the end!  okay??  D’accord??

One more small point – there is absolutely nothing wrong with “pouvez-vous” – but in order to impress the examiner – try to use the future tense (pourrez-vous)  or the conditional tense (pourriez-vous).
